Telecare Registered Nurse – Baby Place

About the role

The Telecare Registered Nurse provides remote triage and monitoring for patients in the Baby Place unit, supporting emergency department diversion and readmission reduction initiatives. Working under the manager’s supervision, the RN delivers evidence‑based care via inbound and outbound phone lines, ensuring timely interventions for out‑of‑range RPM values.

Key responsibilities

• Triages and prioritizes patient calls using evidence‑based practice standards.
• Monitors remote patient data and initiates interventions when values fall outside set parameters.
• Provides telephone advice, patient education, and care plans according to approved protocols.
• Documents all interactions promptly in the electronic health record.
• Coordinates follow‑up calls with caregivers and communicates critical information to physicians.
• Supports nurse case management, conducts nursing assessments, and collaborates with multidisciplinary teams.
• Maintains and troubleshoots telecare equipment to ensure uninterrupted service.

Required profile

• Current Registered Nurse (RN) license.
• Experience with telecare, remote patient monitoring, or telephone triage.
• Strong knowledge of growth and development across the lifespan.
• Ability to assess patient conditions, prioritize care, and document accurately.
• Excellent communication skills and ability to work autonomously while adhering to organizational values.
