Social Work Care Management

About the role

AdventHealth is hiring a Social Work Care Management professional for its Tavares, Florida facility. The role supports patients' discharge planning, care transitions and social needs through assessment, advocacy and coordination with post-acute care and community resources. The schedule is full time, day shift (1:00 PM to 11:30 PM) with rotating weekends.

Key responsibilities

• Provide grief counseling, disease adjustment support, crisis intervention, goals of care planning support and de-escalation services for patients as appropriate.
• Assess patients and families holistically for discharge planning needs in inpatient, observation and emergency settings, covering prior functioning, support systems, finances and psychosocial factors.
• Review medical records and incorporate clinical, social and financial factors into the transition of care plan, developing discharge plans with contingency plans and arranging post-acute and community care.
• Use technology and best practices to coordinate with post-acute providers so records are complete and discharge reconciliation is accurate.
• Take part in multi-disciplinary rounds to review patient status, level of care and discharge plans, escalating care delays to leadership when needed.
• Educate patients and families about the emotional, social and financial impact of illness, mobilize family and community resources and advocate for patient choice and rights.
• Organize and facilitate patient and family care conferences with the multidisciplinary team.
• Document discharge planning evaluations, assessments, discharge plans, MDRs, barriers to care, avoidable days and patient needs.
• Communicate with payers about authorization for post-acute care and assess readmitted patients for their reasons for readmission.

Required profile

• Master's degree (field of study not specified).
• 2+ years of social work experience (required); 2+ years of care management experience preferred.
• Psychosocial assessment skills, critical thinking, problem-solving, organizational and data analysis skills, and strong interpersonal, negotiation and customer service skills.
• Ability to work with people of all backgrounds and adapt to a complex, changing healthcare environment.
• Current working knowledge of discharge planning, utilization management, care management, performance improvement and managed care reimbursement, and of state and federal care management guidelines (preferred).
• Understanding of pre-acute and post-acute care venues and community resources, especially for patients with limited payment resources.
• Preferred: Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification.

Required skills

• Computer proficiency with Outlook e-mail and electronic medical records.
• Knowledge of state and federal care management guidelines (preferred).

The pay range is $23.94 to $44.53. Florida Level 2 background screening applies to certain positions, and additional licensure or certification requirements may depend on the unit or state.
