Social Work Aging Life Care Manager

About the role

We are seeking a licensed social worker to join our Aging Life Care Management practice. This position offers flexible scheduling, the chance to build long‑term relationships with clients and families, and a collaborative, supportive team environment.

Key responsibilities

• Conduct client assessments and develop individualized care plans.
• Visit clients in their homes to support ongoing care needs.
• Communicate regularly with families and healthcare providers.
• Provide guidance during transitions or periods of change.
• Offer occasional weekend on‑call support with eligibility for bonus pay.

Required profile

• Experience working with older adults, home health, hospice, or related fields.
• Highly organized and able to manage multiple moving parts.
• Comfortable using technology such as Google Workspace and databases.
• Strong relationship‑building and thoughtful communication skills.
• Demonstrated emotional intelligence and ability to maintain healthy boundaries.
