Social Services Care Coordinator

About the role

The Social Services Care Coordinator supports clients at a substance‑use disorder treatment facility by managing their ancillary health and social needs. This full‑time, onsite position ensures timely access to medical services, benefits, and transportation while maintaining accurate documentation.

Key responsibilities

• Ensure TB tests are completed within 7 days of admission.
• Apply for state disability and Medi‑Cal benefits on behalf of clients.
• Arrange and provide transportation to medical appointments, courts, and DPSS.
• Assist clients in enrolling with primary care physicians and schedule physical examinations.
• Maintain professional relationships with networking agencies and act as liaison for medical issues.
• Obtain medication approvals and monitor ongoing medical concerns.
• Review and update medication records and document all care coordination in the EHR.
• Report client illnesses, accidents, and injuries to the Director of Nursing.

Required profile

• Strong organizational skills to manage multiple client needs simultaneously.
• Ability to communicate effectively with clients, healthcare providers, and external agencies.
• Attention to detail for accurate documentation and medication record keeping.
• Willingness to work in a 24/7 treatment environment.
