Senior Living Visits Physical Therapist

About the role

BAYADA Home Health Care is seeking experienced Physical Therapists to provide in‑home rehabilitative care for adult and geriatric clients in senior living facilities throughout Montgomery County, MD. This full‑time position involves traveling to client homes and facilities to deliver skilled therapy services.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests, range‑of‑motion measurements, strength and balance assessments, and safety evaluations.
• Develop and implement individualized care plans that include therapeutic exercises, gait training, balance work, and wound‑care interventions.
• Continuously reassess and update therapy plans and collaborate with nursing staff on multidisciplinary care.
• Educate clients and families on rehabilitative activities to promote independence and safety.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current Maryland Physical Therapist (PT) license.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and solve problems.
• Strong interpersonal and organizational skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Competence in performing diagnostic mobility and balance assessments.
