Senior Living Physical Therapist

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide outpatient visits for senior living clients in the Windsor County area of Vermont.

Key responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including range of motion, strength, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapeutic exercises, gait training, and balance interventions.
• Continuously assess and revise care plans and collaborate with nursing as part of a multidisciplinary team.
• Educate clients and families on rehabilitative care to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current Vermont Physical Therapist license.
• Minimum one year of recent clinical experience as a licensed Physical Therapist.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills, including use of tablet devices for documentation.
