Senior Case Manager – Behavioral Health & Substance Use Disorder

About the role

Wise Path Recovery Centers is seeking an experienced, compassionate Senior Case Manager to support individuals navigating substance use disorders and behavioral health challenges. The role coordinates care across clinical, medical, psychiatric, recovery, and community services to ensure seamless treatment and successful transition into recovery.

Key responsibilities

• Conduct biopsychosocial assessments and contribute to individualized treatment planning and ongoing updates.
• Provide comprehensive case management for clients with moderate to high psychosocial, behavioral health, and recovery support needs.
• Coordinate care across clinical, medical, psychiatric, recovery support, and community‑based services.
• Identify barriers to successful treatment and recovery and collaborate with clients and team members to address them.

Required profile

• Demonstrated experience in case management for behavioral health and substance use disorders.
• Highly organized with strong relationship‑building abilities.
• Passionate about helping individuals overcome barriers and achieve sustainable recovery.
