RN Nurse Navigator - Cardiovascular Services

About the role

AdventHealth in Ocala, Florida is hiring a Registered Nurse Nurse Navigator for its cardiovascular services. The nurse coordinates scheduling, supports high-risk cardiovascular patients, acts as a liaison between patients and physicians, and helps improve the quality of cardiovascular care.

Key responsibilities

• Facilitate timely scheduling with coordinators, supervisors, physicians and patients
• Monitor high-risk cardiovascular patients and provide necessary support
• Act as a liaison to physicians' offices on orders, medical records, consents and other documentation
• Coordinate multidisciplinary cardiovascular services including cardiology, cardiothoracic surgery and vascular surgery
• Develop and implement evidence-based measures to improve cardiovascular care quality and ensure regulatory compliance
• Support data collection for process and quality improvement initiatives
• Manage patient access and scheduling, provide patient education and communicate patient status to the care team

Required profile

• Registered Nurse (RN) license
• Associate's or Bachelor's degree in nursing (Master's preferred)
• 2+ years of acute care experience and 2+ years in cardiac care preferred
• Previous leadership experience, project or program start-up experience, and computer skills (Word, Access, Excel, Outlook) preferred
• Florida Level 2 background screening applies

Required skills

• Effective verbal and written communication and ability to explain complex information clearly
• Knowledge of PACS, Heartlab, LumeDx, Cerner and associated systems
• CPR certification (BLS); ACLS preferred
