Job description
About the role
Activate Care is looking for a Registered Nurse Manager to lead and develop a team of RN Supervisors, each overseeing their own distributed team of RN Home Health Assessors who travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. The role focuses on building supervisory capability, standardizing clinical quality and meeting the health plan's completeness, timeliness and quality standards. It is a leadership position with a clean handoff to the health plan's care management team.
Key responsibilities

- Build bench strength and succession readiness, and ensure each Supervisor has a coverage plan for PTO, leave and turnover.
- Set and reinforce field-safety standards for independent in-home visits.
- Partner with the Quality Assurance & Improvement Specialist on audit strategy, including pre-submission audits of assessments and SOAP notes, and monitor documentation turnaround in GuidingCare and completeness/quality performance.
- Analyze error trends, direct corrective action, retraining and calibration, and oversee timely responses to audit findings.
- Act as subject-matter expert on the assessment process, tools and regulatory, privacy (HIPAA), NCQA and care management standards.
- Own caseload distribution, visit scheduling and route planning, track and report program KPIs, and partner with the scheduling team on capacity planning.
- Represent the program to the health plan's program leadership, and oversee compliance, including workforce exclusion checks and fraud, waste and abuse and HIPAA training.
- Escalate and resolve clinical, compliance or client concerns beyond the Supervisor level.
Required profile
- Active RN or higher licensure in good standing with the Commonwealth of Massachusetts.
- Bachelor of Science in Nursing (BSN) required.
- Minimum 5 years of RN experience, including home health or community-based assessment work.
- Minimum 2 years supervising RN Supervisors, team leads or a multi-team/multi-site nursing operation.
- Ability to coach supervisors into leadership and strong quality-assurance and audit experience.
- Experience with electronic platforms via a secure virtual desktop (GuidingCare) and leading a remote, field-based, multi-team organization.
- Experience auditing assessments (UCA, FA, Comprehensive, MDS, OASIS); RAC-CT certification preferred.
- Prior experience with dual-eligible populations, Medicaid or Medicare managed care preferred.
- Must live within commuting distance of the assigned Massachusetts territory, hold a valid driver's license, vehicle and verifiable insurance, and be vaccinated against influenza during flu season.
