RN Case Manager – Care Ally

About the role

The RN Case Manager – Care Ally works in collaboration with physicians, patients and families to promote quality outcomes for patients with complex medical needs. This full‑time, 40‑hour per week position is based at Duly Health and Care in Illinois and involves coordinating care across multiple settings to prevent readmissions and meet patients’ medical, environmental and psychosocial needs.

Key responsibilities

• Analyze data from predictive modeling tools to identify eligible patients for the Case Management Program.
• Enroll and manage a caseload of patients with complex medical needs, completing assessments and attestations per health‑plan requirements.
• Develop individualized case management plans with prioritized goals and appropriate interventions.
• Coordinate health‑care services with physicians and providers, ensuring appropriate level of care and preventing inpatient readmissions.
• Maintain comprehensive documentation, member communications, and case closures in accordance with policies.
• Provide resources and advocacy for patients, families, and utilization‑management staff.

Required profile

• Active Registered Nurse license in Illinois, in good standing.
• 2–4 years of RN experience in primary care or hospital settings; case‑management experience preferred.
• Associate’s degree in nursing required; Bachelor’s degree preferred.
• Current BLS/CPR certification.
• Certification in Case Management preferred.
