RN Case Manager – Benton/Bryant

About the role

The RN Case Manager provides comprehensive case management services for patients at UAMS Health locations in Benton and Bryant. This role supports coordinated care, ensures appropriate resource utilization, and enhances patient outcomes through effective communication and clinical expertise.

Key responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate interdisciplinary services and follow‑up appointments.
• Monitor patient progress, adjust plans as needed, and document outcomes.
• Educate patients and families about treatment plans, discharge instructions, and community resources.
• Collaborate with physicians, social workers, and other healthcare professionals to ensure seamless care transitions.

Required profile

• Current Registered Nurse (RN) license in the United States.
• Eligibility to work in the United States on the first day of employment.
• Strong communication and interpersonal skills.
• Commitment to patient‑centered care and teamwork.
