Remote Nurse Practitioner – Telehealth Urgent Care

About the role

We are seeking a compassionate Nurse Practitioner to join our remote telehealth team, providing urgent‑care support to Medicare beneficiaries enrolled in our Chronic Care Management program. You will evaluate non‑life‑threatening clinical escalations, guide treatment, and help keep members out of the emergency department.

Key responsibilities

• Conduct telehealth evaluations for urgent, non‑emergent concerns.
• Review symptoms and medical histories to determine appropriate interventions.
• Escalate to primary care, emergency services, or 911 when clinically indicated.
• Document all encounters accurately in the electronic medical record.
• Reduce avoidable ED visits and hospitalizations through proactive care.
• Educate patients on disease management, medication adherence, lifestyle changes, and symptom monitoring.
• Provide clinical guidance to nursing staff on escalation pathways.
• Participate in case reviews and quality‑improvement initiatives.

Required profile

• Active, unrestricted Nurse Practitioner license.
• National certification appropriate to your specialty.
• Minimum three years of clinical practice, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.
