Remote Nurse Practitioner – Telehealth Urgent Care

About the role

We are seeking a compassionate Nurse Practitioner to join our remote telehealth team, providing urgent‑care support to Medicare beneficiaries enrolled in our Chronic Care Management program. You will assess members with non‑life‑threatening issues, deliver evidence‑based recommendations, and help keep patients out of the emergency department.

Key responsibilities

• Conduct telehealth evaluations for urgent, non‑emergent clinical concerns.
• Review symptoms and medical histories to determine appropriate interventions.
• Escalate care to primary providers, emergency services, or 911 when clinically indicated.
• Document all encounters accurately in the electronic medical record.
• Assist in reducing avoidable ED visits and hospitalizations through proactive clinical intervention.
• Provide patient education on disease management, medication adherence, lifestyle modifications, and symptom monitoring.
• Offer clinical guidance to nursing staff on escalation pathways.
• Participate in case reviews and quality‑improvement initiatives.

Required profile

• Active, unrestricted Nurse Practitioner license.
• National certification appropriate to specialty.
• Minimum of 3 years of clinical practice experience, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.
