Remote Case Manager RN (Full‑time)

About the role

The University of Miami UHealth Department of UMHC‑SCCC Business Operations is seeking a full‑time Remote Case Manager RN. The incumbent coordinates interdisciplinary care plans from admission to discharge, acts as a liaison among patients, families, healthcare personnel, and insurers, and develops discharge plans that align resources with patient needs.

Key responsibilities

• Identify patient risk factors, obstacles to care, and readmission risk.
• Evaluate care plans through chart reviews and patient interviews, collaborating with the medical team.
• Educate patients and families on care progression.
• Serve as liaison to ensure timely, effective, and fiscally responsible care.
• Promote quality care and appropriate service utilization.
• Facilitate insurance authorizations and regulatory notifications.
• Maintain knowledge of insurance reimbursement policies.
• Plan and execute discharge and transition plans meeting physical, social, and emotional needs.

Required profile

• Bachelor’s degree.
• Active Registered Nurse (RN) license.
• Minimum 2 years of relevant nursing experience.
• Strong oral and written communication skills.
• Ability to analyze, prioritize, and solve problems under pressure.
