About the role
Vivo Care is seeking a licensed nurse to serve as a Care Navigator, providing remote support to adult and geriatric patients. You will use motivational communication to triage vital signs, promote healthy behaviors, and coordinate care through our Remote Care Platform.
Key responsibilities
- Monitor physiological markers such as blood pressure, weight, and blood glucose via the platform.
- Conduct wellness calls to set patient‑centric goals and develop action plans.
- Collaborate with the patient’s care team to ensure appropriate clinical interventions.
- Maintain compliance with reimbursement requirements and meet team performance metrics.
Required profile
- Licensed nurse with experience caring for adult and geriatric populations.
- Background in phone triage and remote‑patient monitoring preferred.
- Strong teamwork skills and ability to work independently in a remote setting.
