About the role

VNS Health is seeking a Spanish‑speaking Registered Nurse to provide home‑based, patient‑centered care across New York State. You will work independently within an interdisciplinary team, delivering personalized nursing and care‑management services directly in patients’ homes or care facilities.

Key responsibilities

• Practice independently in the community as part of an interdisciplinary care team.
• Deliver personalized nursing and care management to patients in their homes or care facilities.
• Continuously assess evolving patient needs and adjust care plans accordingly.

Required profile

• Current Registered Nurse license in New York State.
• Minimum one year of nursing experience in a medical/surgical environment.
• Valid driver’s license or NYS Non‑Driver photo ID (may be required).
