Registered Nurse – Patient Support Team

About the role

Equip is seeking a Registered Nurse to join its virtual, evidence‑based eating‑disorder treatment program. The RN will work remotely, providing day‑to‑day collaborative support to internal medical and psychiatric providers and ensuring seamless patient care across multiple touchpoints.

Key responsibilities

• Serve as the primary liaison for a panel of medically stable patients.
• Monitor and triage lab orders and results, communicating provider instructions to patients and families.
• Manage the RN phone line to coordinate care with external providers.
• Follow established triage protocols, answering patient and family inquiries.
• Provide condition‑specific education and medical management support to families.
• Handle emergent issues promptly in line with triage guidelines.
• Coordinate hospital admissions for medical stabilization and ensure smooth transitions back to virtual care.
• Complete accommodation paperwork and conduct solo patient visits when needed.

Required profile

• Current, unrestricted RN license in at least one U.S. state (compact license preferred).
• Minimum 2 years of nursing experience in a clinical, home‑care, ambulatory, hospital, or inpatient setting.
• Proven ability to coordinate care among multiple providers and support systems.
• Strong organizational skills with the capacity to prioritize multiple requests.
• Excellent communication skills and strict adherence to confidentiality.
• Experience working on multidisciplinary teams and a collaborative mindset.
• Ability to work autonomously within defined clinical protocols.
