Registered Nurse Home Health - Nurse Case Manager

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning and managing ongoing outpatient care, under the supervision of an RN or MD. The position is open to active, licensed RNs in the Fayetteville, NC area.

Key responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other disciplines or document the rationale for not doing so
• Promote patient health and independence through teaching and rehabilitative measures, helping patients learn self-care techniques
• Supervise LPNs and HHAs
• Complete documentation timely and accurately, and share patient progress with the clinical manager and care team
• Plan and provide staff education, and perform on-call duties as assigned
• Take part in clinical development and continuing education programs

Required profile

• Current RN license for the state(s) of assignment
• At least 1 year of clinical experience as a Registered Nurse
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

What we offer

• Sign-on bonus of $15K
• Total cash pay of $61,153 to $91,729 annually, plus $35.28 to $52.92 per visit point and an hourly rate of $29.40 to $44.10
