Registered Nurse Case Manager – Per Diem

About the role

The University of Miami is seeking a Per Diem Registered Nurse Case Manager to coordinate interdisciplinary care plans from admission through discharge. You will act as a liaison among patients, families, healthcare teams, and insurers to ensure optimal outcomes.

Key responsibilities

• Develop and monitor individualized care plans and discharge strategies.
• Communicate with patients, families, clinicians, and insurers to facilitate timely care.
• Identify risk factors, obstacles to care, and readmission risks.
• Conduct chart reviews and patient interviews to evaluate care plans.
• Educate patients and families on care progression and post‑discharge needs.
• Obtain and maintain insurance authorizations and regulatory documentation.
• Suggest improvements to clinical pathways and quality of care.

Required profile

• Graduate from an accredited nursing program with a BSN.
• Valid Florida Registered Nurse license.
• BLS certification from the American Heart Association.
• Minimum 2 years of nursing experience, including at least 1 year in hospital case management.
• Strong knowledge of patient assessment and medical terminology.

Required skills

• Basic Life Support (BLS) certification
• Florida RN licensure
