Registered Nurse Case Manager – Lead Unit

About the role

The City of Newark is seeking a Registered Nurse to join the Division of Environmental Health’s Childhood Lead Unit. Under supervision, the RN will conduct home visits, provide case‑management services, and coordinate lead‑poisoning prevention for children.

Key responsibilities

• Perform home visits to assess health, develop comprehensive care plans, and provide counseling on lead‑poisoning prevention.
• Monitor blood‑lead levels, arrange retesting, and refer families to immunization clinics and other services as needed.
• Coordinate lead screening for siblings, arrange emergency relocation funding, and assist families with cleaning supplies.
• Utilize electronic health record systems, including Welligent, to document case progress and close cases when objectives are met.

Required profile

• Current, valid RN license in New Jersey.
• Valid NJ driver’s license and access to a vehicle for home visits.
• Knowledge of nursing standards, immunization protocols, and medical laboratory procedures.
• Ability to analyze case data, maintain cooperative relationships, and prepare detailed case reports.

Required skills

• Proficiency with Electronic Health Record systems (Welligent).
• Venipuncture and basic laboratory testing.
