Registered Nurse Care Management

About the role

AdventHealth in Orlando is hiring a full-time Registered Nurse for its Care Management team. The schedule consists of five 8-hour shifts per week (40 hours), with a requirement of four weekend shifts in every six-week period.

Key responsibilities

• Take part in multi-disciplinary rounds reviewing patient status, progression, level of care and discharge plans, and escalate care delays to leadership
• Educate patients and families on the emotional, social and financial impact of illness and help them access community resources
• Assess readmitted patients for their reasons for readmission
• Organize and run patient and family care conferences with the multidisciplinary team
• Document discharge plans, assessments, barriers to care and patient needs, and coordinate with post-acute care providers so records are complete and discharge reconciliation is accurate
• Perform other duties as assigned

Required profile

• Associate degree in nursing (Bachelor's preferred)
• 2+ years of medical or hospital nursing experience
• Registered Nurse license and Basic Life Support certification (preferred)
• Care Management or Utilization Management experience preferred
