Registered Nurse – Admissions & Discharge (Full‑Time)

About the role

The University of Miami is seeking a full‑time Registered Nurse to lead admission, transfer, and discharge processes. You will serve as a key liaison between patients, families, and the interdisciplinary care team, ensuring safe and coordinated transitions of care.

Key responsibilities

• Apply the nursing process to assess, plan, and meet patient and family needs during admission, transfer, and discharge.
• Review all orders for new admissions, transfers, and discharges.
• Communicate patient status changes to the care team and provide timely updates to patients and families (within 24‑72 hours of admission and 24 hours post‑discharge).
• Participate in daily interdisciplinary rounds and collaborate with primary RN, case managers, and social services.
• Coordinate proactive discharge planning, schedule follow‑up appointments, and ensure medication reconciliation.
• Provide patient and family education, validate comprehension, and document discharge teaching.
• Conduct post‑discharge follow‑up calls at 3, 7, 14, and up to 30 days as needed.
• Maintain a safe, clean environment and comply with regulatory standards and National Patient Safety Goals.

Required profile

• Active RN license in Florida.
• Experience with admission, transfer, and discharge processes in an acute care setting.
• Strong knowledge of the nursing process and interdisciplinary collaboration.
• Excellent communication skills for interacting with patients, families, and multidisciplinary teams.
• Commitment to patient education and quality improvement initiatives.
