Primary Care Physician – Value‑Based Medicare Advantage

About the role

ChenMed is seeking experienced Primary Care Physicians to join its rapidly expanding, physician‑led network that delivers full‑risk Medicare Advantage care to seniors. Physicians will manage a panel of up to 450 patients, focusing on outcomes, cost‑effectiveness, and personalized health coaching.

Key responsibilities

• Provide comprehensive primary care for a defined patient panel, ensuring high‑quality clinical outcomes.
• Act as a health coach, helping patients set and achieve short‑ and long‑term health goals.
• Utilize a value‑based metric system, tracking patient admissions per thousand, appointment slot utilization, CGCAHPS scores, clinical gap closures, and medical cost measures.
• Collaborate with care teams, medical directors, and managers to meet performance targets and maintain compliance with clinical guidelines.
• Participate in risk‑management, quality‑improvement programs, and regular clinical meetings.
• Champion a positive culture of love, accountability, and passion within the medical center.

Required profile

• Board‑certified or board‑eligible physician with a strong work ethic and entrepreneurial mindset.
• Demonstrated ability to build trusting relationships with patients and influence health behaviors.
• Comfortable working both independently and as part of multidisciplinary teams.
• Commitment to delivering affordable, high‑quality care to senior populations.
