Physician (MD/DO) – Primary Care Provider

About the role

Northwest Human Services is seeking a board‑eligible or board‑certified MD or DO to join its West Salem Clinic as a primary care physician. You will work in an outpatient community health center serving uninsured, homeless, migrant, and LGBTQIA+ populations, providing culturally and linguistically appropriate medical care.

Key responsibilities

• Provide comprehensive examinations, diagnoses, and treatment plans for adult patients.
• Coordinate patient care, including specialty referrals, follow‑up, and case management.
• Collaborate with community health workers, nurses, mental‑health clinicians, and other team members to ensure continuity of care for complex needs.
• Participate in integrated medical‑behavioral health model to address both physical and mental health factors.

Required profile

• MD or DO degree with licensure to practice in Oregon.
• Training in Family Medicine or Internal Medicine.
• Commitment to serving underserved and diverse populations.
• Flexibility to work part‑time or full‑time within clinic hours.

• Competitive salary ranging from $212,888 to $319,384 (DOE).
