Physical Therapist – Jacksonville

About the role

The Physical Therapist will provide comprehensive assessment, planning, evaluation and implementation of physical therapy care for assigned patients in Jacksonville. This role ensures compliance with the Physical Therapy Practice Act and delivers high‑quality therapeutic interventions, equipment, and supplies to meet patient needs.

Key responsibilities

• Gather relevant patient data from charts and other caregivers, consulting physicians when necessary.
• Independently assess all assigned patient types and identify additional needs during evaluation.
• Make informed decisions regarding additional testing, scheduling, and performance of tests.
• Interpret assessment results and develop measurable, realistic, and attainable treatment goals.
• Reassess patient progress regularly in line with departmental and regulatory policies.
• Recommend and discuss appropriate therapeutic equipment with patients and families.

Required profile

• Doctorate in Physical Therapy preferred; Bachelor’s degree required.
• Current LPT (Licensed Physical Therapist) license in the state of employment.
• Current BLS (Basic Life Support) certification.
• Ability to work independently and exercise sound clinical judgment.
