Physical Therapist - Home Health

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. Its Physical Therapist - HAC helps patients restore function, improve mobility and enhance quality of life, providing skilled physical therapy under a physician-directed plan of care within an interdisciplinary model.

Key responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care with physicians
• Provide skilled physical therapy interventions and adjust treatment plans based on patient progress and clinical needs
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination
• Collaborate with interdisciplinary teams to coordinate care and share patient progress and condition changes
• Maintain accurate, timely documentation in line with clinical, regulatory and documentation standards

Required profile

• Current, unencumbered license to practice physical therapy in the assigned state
• Graduate of an accredited physical therapy program (CAPTE accredited)
• 1+ year of physical therapy experience, or less than one year with an approved residency or mentorship program
• 1+ year of experience in clinical assessment, documentation and care coordination
• Current CPR certification
• Driver's license and access to reliable transportation
• Preferred: 1+ year in home health, hospital-at-home or community-based therapy, and 1+ year in interdisciplinary healthcare teams

• Salary range of $106,400 to $159,600 annually for full-time employment
