Pharmacist - Outpatient / HH-Pharmacy

About the role

The Outpatient / HH-Pharmacy Pharmacist will ensure safe, accurate, and appropriate medication use for pediatric patients at Children’s, the region’s only full‑service pediatric healthcare center. The role involves evaluating prescriber orders, supervising pharmacy staff, and providing medication counseling to patients and families.

Key responsibilities

• Review and triage medication orders for dosing, allergies, contraindications, and urgency.
• Supervise supportive pharmacy personnel and verify prepared pharmaceutical products.
• Perform pharmacokinetic monitoring, medication reconciliation at discharge, and report adverse drug events.
• Provide cost‑effective drug therapy recommendations and counseling to patients and families.
• Participate in education of pharmacy students, residents, and clerkship programs.
• Maintain accurate documentation in patient charts and electronic order entry systems.

Required profile

• Current, unrestricted pharmacist license.
• Experience in outpatient or hospital pharmacy settings, preferably pediatric.
• Strong knowledge of drug interactions, dosing calculations, and clinical pharmacology.
• Ability to supervise staff and collaborate with multidisciplinary healthcare teams.
