Pharmacist – Clinical Pharmacy Services

About the role

The pharmacist will ensure safe, timely, and cost‑effective medication therapy for patients by interpreting medication orders, preparing sterile products, and providing clinical consultation to healthcare providers. This role supports quality improvement initiatives and collaborates with inter‑professional teams to optimize medication use.

Key responsibilities

• Interpret medication orders and maintain accurate patient medication profiles.
• Prepare sterile products, including large and small volume parenterals, using aseptic technique and adhering to USP 800 guidelines.
• Select, label, and dispense medications safely in line with policies and procedures.
• Provide drug information, clinical consultation, and evidence‑based therapeutic recommendations to prescribers.
• Implement and maintain clinical pharmacy services, ensuring compliance with regulatory standards.
• Participate in quality improvement, research projects, and medication‑use review activities.
• Document clinical interventions and contribute to treatment guidelines, formulary changes, and critical pathways.

Required profile

• Licensed pharmacist with experience in clinical or hospital pharmacy settings.
• Strong knowledge of medication safety, regulatory standards, and pharmacy best practices.
