Pediatric Nurse Practitioner – In‑Home Preventive Care

About the role

Provide pediatric preventive care directly in families’ homes across Coconino County, Arizona. Set your own schedule and conduct in‑home Well‑Child Visits to identify needs early and close care gaps.

Key responsibilities

• Perform scheduled in‑home Well‑Child Visits throughout Coconino County.
• Review medical history, vitals, risk factors and complete age‑appropriate pediatric assessments.
• Document findings accurately in the EMR and educate families on next steps.
• Identify care gaps and coordinate appropriate follow‑up services.
• Manage your local route independently without daily clinic reporting.

Required profile

• Active Nurse Practitioner license in Arizona.
• PNP‑PC certification or equivalent pediatric NP certification.
• Registered/enrolled with Arizona Medicaid.
• Valid driver’s license and reliable transportation.
• Comfortable working independently in patients’ homes.
