Patient Navigator – Full‑time, Miami

About the role

The University of Miami/UHealth Department of Site Disease seeks a full‑time Patient Navigator to help patients overcome barriers to care, coordinate treatment plans, and ensure follow‑up of services ordered by caregivers.

Key responsibilities

• Conduct patient evaluations to gather information for effective treatment planning.
• Facilitate access to insurance and eligible social service programs.
• Provide appointment reminders and act as liaison between patients and providers.
• Identify and resolve real or perceived barriers to care.
• Educate and counsel patients on health‑related and life‑situational issues.
• Maintain up‑to‑date knowledge of community health and welfare resources.
• Obtain and track medical records, pathology slides, and imaging reports from external facilities.
• Prepare and process patient charts, ensuring confidentiality and compliance with hospital policy.
• Assist nursing staff with multidisciplinary meetings and perform clerical duties such as faxing, scanning, and copying.

Required profile

• High School diploma or equivalent.
• Minimum three years of relevant experience, preferably in a medical office or hospital.
• Ability to handle confidential information with discretion.
• Strong oral and written communication skills.
• Professional composure in stressful situations and effective interpersonal relationships.

Required skills

• Microsoft Office proficiency.
• Familiarity with UChart, RIS, PACS, and Cerner systems.
