About the role
The University of Miami/UHealth Department of Site Disease is seeking a full‑time Patient Navigator to support patients in Miami. The role focuses on removing barriers to care, coordinating treatment plans, and ensuring follow‑up of services ordered by caregivers.
Key responsibilities
- Conduct patient evaluations to gather information for effective treatment planning.
- Assist patients in accessing eligible insurance and social service programs.
- Provide appointment reminders and act as a liaison between patients and providers.
- Identify and resolve real or perceived barriers to care.
- Educate and counsel patients on health‑related and life‑situational issues.
- Maintain up‑to‑date knowledge of community health, welfare resources, and other support programs.
- Handle telephone communications, obtain medical records, and manage consent documentation in compliance with HIPAA.
Required profile
- High school diploma or equivalent.
- Minimum of three years of relevant experience in patient navigation, case management, or a similar role.
- Ability to process confidential information with discretion.
- Strong oral and written communication skills.
- Professional composure in stressful situations and effective interpersonal relationships.
Required skills
- Knowledge of HIPAA regulations and consent procedures.
- Proficiency in telephone etiquette and record‑retrieval processes.
