Patient Navigator – Full‑time

About the role

The University of Miami UHealth at SoLé Mia is seeking a full‑time Patient Navigator to support patients across a range of specialties. In this role you will act as a bridge between patients, providers, and community resources, ensuring timely access to care and eliminating barriers.

Key responsibilities

• Conduct patient evaluations to gather information needed for effective treatment planning.
• Facilitate access to insurance and eligible social service programs.
• Provide appointment reminders and serve as a liaison between patients and healthcare providers.
• Identify and resolve real or perceived barriers to care.
• Educate and counsel patients on health‑related and life‑situational issues.
• Maintain up‑to‑date knowledge of community health and welfare resources.
• Handle patient communications, request medical records, and address care gaps with the nurse navigator.
• Prepare, track, and process medical records and pathology specimens, ensuring completeness and confidentiality.
• Provide administrative support, including answering phones, copying, faxing, emailing, and mailing records.
• Adhere to university policies and safeguard institutional assets.

Required profile

• Experience facilitating patient access to care and coordinating treatment plans.
• Strong understanding of insurance eligibility and social service programs.
• Excellent communication and interpersonal skills for interacting with patients, providers, and community agencies.
• Ability to identify barriers to care and develop practical solutions.
• Detail‑oriented approach to managing medical records and documentation.
