Outpatient Occupational Therapist – Senior Living

About the role

BAYADA Home Health Care is seeking an experienced Outpatient Occupational Therapist to provide home health visits for senior living communities in the Raleigh‑Durham area. This part‑time position supports adult and geriatric patients, helping them remain safely at home.

Key responsibilities

• Make home visits to clients within designated geographic territories.
• Perform comprehensive functional evaluations, including muscle function, endurance, visual coordination, communication skills, ADL/IADL performance, and cognitive/psychological aspects.
• Assess home environments for hazards, recommend equipment and adaptations, and develop individualized intervention plans.
• Continuously revise therapy care plans and collaborate with nursing on multidisciplinary care.
• Educate patients, families, and caregivers on rehabilitative activities to promote safety and independence.
• Document observations, interventions, and evaluations using a tablet‑based electronic system.

Required profile

• Current North Carolina Occupational Therapist license.
• Minimum one year of recent occupational therapy experience.
• Graduation from an accredited OT program (COAHEA/AOTA).
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

• Proficiency with electronic medical record (EMR) systems.
