Occupational Therapist – Home Health

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide home health visits for adult and geriatric patients in senior living communities throughout Charlottesville, VA. The role can be per diem, part‑time, or full‑time.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform comprehensive functional evaluations, including muscle function, endurance, visual coordination, communication skills, ADL/IADL performance, and cognitive/psychological status.
• Assess home environments for hazards, recommend equipment and adaptations, and develop individualized intervention plans.
• Collaborate with nursing and multidisciplinary teams, continuously revise care plans, and educate patients and families.
• Document all observations, interventions, and evaluations using a tablet‑based EMR system.

Required profile

• Current Virginia Occupational Therapist license.
• At least one year of recent OT experience, with prior home‑care experience required.
• Graduation from an accredited OT program.
• Ability to work independently, manage time effectively, and strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills.
