Nurse Surveyor (Health Facilities Surveillance Nurse)

About the role

The Illinois Department of Public Health is seeking a Health Facilities Surveillance Nurse in the Long-Term Care Division, Field Operations, to survey facilities in the Division of Intermediate Care Facilities/Individuals with Intellectual Disabilities. The nurse conducts surveys of facilities, including state licensure, federal certification and complaint surveys, and prepares written survey documentation.

Key responsibilities

• Under general supervision, survey facilities to determine compliance with state licensure requirements, federal certification mandates and department directives, program policies and procedures.
• Travel to conduct routine health and safety surveys to determine compliance with state and federal requirements.
• Interview facility residents, staff, administrators, health care providers and other individuals.
• Observe patient care provided at facilities.
• Review medical records, policies, standard operating procedures and other facility documents related to licensure and regulatory compliance.

Required profile

• Applicants must be currently authorized to work in the United States on a full-time basis; the state does not provide visa sponsorship.
• Bilingual skills are welcome.

What we offer

• Anticipated starting salary of $7,423 per month, with a range of $7,423 to $10,093 per month.
• Full-time, salaried position, Monday to Thursday, 7:30 AM to 5:30 PM (with a half-hour lunch).
• Hybrid availability of 1 to 5 days per week in the office, depending on operational needs.
• Union position, so the collective bargaining agreement applies.
