About the role

Avir Health Group is seeking a Nurse Practitioner to lead clinical care in a Skilled Nursing Facility. You will work closely with the multidisciplinary team to provide high‑quality care for both short‑term and long‑term residents.

Key responsibilities

• Manage admissions and discharges of residents.
• Conduct monthly stability checks and evaluate changes in condition.
• Develop and implement advanced care plans.
• Perform comprehensive assessments and prescribe appropriate treatments.
• Serve as a clinical leader, collaborating with the facility’s care team.

Required profile

• Active Texas Advanced Practice Nurse (APN) licensure and DEA license.
• Previous nursing or clinical experience in a skilled‑nursing setting.
• Strong communication and teamwork abilities.
• Master’s degree in Nursing.
