About the role

Join Optum Home & Community Care as a Nurse Practitioner delivering home‑based medical care to complex, chronic patients. You will work within an interdisciplinary team to improve health outcomes and keep older adults safely at home.

Key responsibilities

Perform comprehensive assessments for newly enrolled patients and develop individualized care plans.
Prescribe diagnostics and interventions, conduct follow‑up and post‑discharge assessments, and coordinate care with primary care providers and specialists.
Provide on‑call support during business hours and participate in a rotational after‑hours on‑call program.
Educate patients and caregivers on disease management, wellness, and care plans.

Required profile

Licensed advanced practice nurse or physician assistant meeting state practice regulations.
Experience delivering home‑based or community health care and collaborating with interdisciplinary teams.
