Nurse Navigator – Patient Care Coordination

About the role

The University of Miami’s Department of Interventional Radiology is seeking a full‑time Nurse Navigator 2 to guide patients and families through the diagnostic and treatment journey. The role focuses on education, scheduling, and continuous support across the care continuum.

Key responsibilities

• Schedule new patient appointments with appropriate providers and ensure timely initial visits.
• Review external medical records to determine appropriate scheduling and care pathways.
• Identify barriers to care and refer patients to support services and relevant organizations.
• Educate patients on their diagnosis, treatment plan, and positive health practices.
• Coordinate communication between patients, families, caregivers, and the multidisciplinary care team.
• Manage Interventional Radiology APP communications, including emails, calls, and secure chats.
• Prepare appeal letters and facilitate peer‑to‑peer reviews for procedure authorizations.
• Conduct outreach to new referrals, schedule visits, and ensure seamless onboarding.
• Participate in multidisciplinary care planning and follow‑up.
