Nurse Navigator – Outpatient Cardiology

About the role

The University of Miami Outpatient Cardiology Clinic is seeking a Nurse Navigator to serve as the first point of contact for patients entering the health system. The role ensures timely scheduling, coordinates care across the continuum, and acts as a liaison among patients, families, and the multidisciplinary team.

Key responsibilities

• Schedule new patient appointments with appropriate providers and ensure timely initial visits.
• Review external medical records to determine appropriate scheduling.
• Identify and address barriers to care, referring patients to support services and community resources.
• Educate patients on their treatment plans based on diagnosis.
• Support patients throughout the care continuum and counsel on positive health practices.
• Collaborate with a multidisciplinary team to outline optimal treatment strategies.
• Mentor and precept students, new graduates, and visiting professionals.
• Maintain professional knowledge through participation in continuing education and professional organizations.

Required profile

• Bachelor’s degree in Nursing.
• Valid State of Florida RN license.
• Minimum of 2 years relevant nursing experience.
• Commitment to quality patient care and ability to implement innovative solutions.
