MTM Clinical Pharmacist

About the role

Clearway Health is seeking an MTM Clinical Pharmacist to ensure optimal patient outcomes for chronic and specialty drug‑related disease states. You will serve as a medication expert for patients, families, and healthcare teams while supporting education and drug acquisition activities.

Key responsibilities

• Conduct comprehensive medication therapy reviews for specialty medication patients, ensuring safety, efficacy, and adherence.
• Provide disease‑specific counseling and education to patients and families.
• Partner with prescribers, nurses, case managers, and payors to initiate and maintain specialty therapies.
• Support prior authorization, appeals, and benefits verification to secure timely patient access.
• Monitor clinical outcomes, adverse events, and adherence metrics.
• Document interventions and outcomes for quality improvement and compliance.
• Ensure activities meet URAC/ACHC standards, manufacturer agreements, and payor requirements.
• Participate in developing clinical protocols, therapeutic management programs, and formulary strategies.

Required profile

• PharmD or bachelor's degree from an accredited College of Pharmacy.
• PGY‑1 residency or 3+ years as a clinical pharmacist in direct patient care.
• Active pharmacist licensure in the state of employment (or ability to obtain at hire).
• Ability to obtain a State Board of Pharmacy Preceptor’s license within one year of hire.

Required skills

• Proficiency in Microsoft Office (Word, Excel, Outlook).
• Knowledge of URAC/ACHC specialty pharmacy standards and compliance documentation.

What we offer

• Competitive annual salary range $146,000 – $182,000 USD.
