Licensed Practical Nurse (LPN) – Full-Time & PRN

About the role

Wise Path Recovery Centers in Williamson, WV is seeking a Licensed Practical Nurse (LPN) to join a multidisciplinary team dedicated to substance‑use and mental‑health recovery. The LPN provides compassionate, person‑centered nursing care under RN or nursing leadership supervision, supporting patients throughout their treatment journey.

Key responsibilities

• Deliver direct nursing care and support under RN or nursing leadership direction.
• Conduct basic nursing assessments and promptly report significant findings.
• Administer medications and treatments in line with physician orders and facility protocols.
• Monitor vital signs, withdrawal symptoms, and overall patient stability.
• Observe patients for condition changes and escalate concerns appropriately.
• Assist with patient education, discharge planning, and care coordination.
• Maintain accurate Medication Administration Records (MARs) and manage medication inventory.
• Participate in interdisciplinary treatment team meetings and communicate clinical observations.

Required profile

• Current, unrestricted LPN license in West Virginia.
• Ability to work under the supervision of an RN, Nurse Manager, or Director of Nursing.
• Strong clinical judgment and attention to patient safety.
• Willingness to work 12‑hour shifts on a 3‑day workweek, with day or night options.

Required skills

• Medication administration according to orders.
• Vital signs monitoring and withdrawal symptom assessment.
• Accurate documentation in Medication Administration Records (MARs).
• Basic patient assessment techniques.
• Inventory management for medications.
