Intake Coordinator (Care Coordinator) – Full-Time

About the role

The Intake Coordinator (also referred to as Care Coordinator) joins Renewal Falls Recovery Center to guide patients through every stage of their treatment journey, from admission to discharge. This role ensures that care is seamless, barriers are addressed, and patients are linked to the resources they need for lasting recovery.

Key responsibilities

• Support admission and intake, including patient orientation, documentation, consents, and releases.
• Assist with initial screenings and assessments within the scope of the position.
• Identify patient needs, strengths, barriers, and available supports.
• Coordinate care across clinical, medical, nursing, peer support, behavioral health, and community providers.
• Participate in interdisciplinary treatment team meetings and communicate updates on patient progress.
• Help develop and review treatment plans and patient goals.
• Facilitate referrals to healthcare providers, outpatient services, recovery housing, peer support, and community resources.
• Assist patients in accessing housing, transportation, employment, insurance, benefits, and legal resources.

• Excellent organizational abilities to manage multiple referrals and documentation.
• Commitment to compassionate, patient‑centered care in a behavioral health setting.
• Ability to work flexible hours, including occasional evenings or weekends.
