About the role

James River Home Health is seeking a PRN Hospice Registered Nurse to deliver personalized, one‑on‑one care to homebound patients in Winchester and surrounding areas. You will work independently in patients' homes, promoting independence and enhancing quality of life.

Key responsibilities

Administer nursing care and skills to homebound patients according to the Plan of Care.
Monitor and report changes in patient symptoms or behavior, and manage multi‑disciplinary care continuity.
Communicate with the interdisciplinary team, educate patients and families, and document visits using Homecare Homebase.

Required profile

Current Registered Nurse license in Virginia.
At least one year of home health experience (preferred).
Valid driver’s license and reliable vehicle for work hours.

Required skills

Experience with OASIS assessments.
Familiarity with Homecare Homebase (HCHB) electronic charting.
