Home Health Visits Registered Nurse

About the role

BAYADA Home Health Care is seeking a Registered Nurse to join its home health visit team in the Middleboro, Lakeville, and Carver area. You will provide skilled nursing and rehabilitative care in patients’ homes, helping them stay safely at home.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Provide acute and post‑acute care including CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document wound assessments, care, and other clinical information using point‑of‑care technology.

Required profile

• Current Massachusetts RN license in good standing.
• Graduated from an accredited nursing program.
• At least one year of recent hands‑on experience with adult/geriatric patients in acute, emergency, home health, hospice, ICU, med‑surg, sub‑acute, or rehab settings.
• Ability to work independently, make real‑time clinical decisions, and prioritize a dynamic visit schedule.
• Strong problem‑solving skills and cultural sensitivity.
