About the role

Carolina Home Care, a part of LHC Group and a member of the Optum family, is hiring a Physical Therapist for home health in Forest City, North Carolina. The therapist assesses patients' functional status, mobility, endurance, pain and other medical conditions, helps set a treatment plan, delivers interventions and evaluates progress. A $15,000 sign-on bonus is offered.

Key responsibilities

• Evaluate patients' functional status and physical therapy needs, and assist the physician in developing the plan of care
• Provide services within the scope of practice allowed by state law, in line with the plan of care and with the health care team
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician
• Treat patients to relieve pain, restore function and maintain maximum functioning
• Instruct patients, families and caregivers about the plan of care, functional limitations and progress
• Visit patients according to the plan of treatment, complete a progress note for each visit and submit them per LHC policy
• Complete all patient evaluations, develop the PT plan of care within state guidelines and report outcomes, goals and expected frequency of care

Required profile

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, access to a dependable vehicle or public transportation
• Ability to work in any home situation regardless of the client's background

What we offer

• $15,000 sign-on bonus
• Annual target pay of $82,048 to $123,072; hourly pay of $39.45 to $59.17; per-visit point pay of $47.34 to $71.00
