Home Health Occupational Therapist (Full‑Time)

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide in‑home rehabilitative care for adult and geriatric patients in Stokes and Surry counties, North Carolina. This full‑time position is part of a multidisciplinary team delivering skilled nursing and therapy services.

Key responsibilities

• Conduct home visits within the designated geographic territory.
• Perform comprehensive functional evaluations, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects and develop individualized intervention plans.
• Evaluate the home environment for hazards and recommend equipment or adaptations.
• Continuously revise the occupational therapy care plan and collaborate with nursing staff.
• Educate patients, families, and caregivers on rehabilitative activities to promote safety and independence.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current North Carolina Occupational Therapist license.
• Minimum one year of recent occupational therapy experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Solid computer skills, including use of tablet devices for documentation.
