Home Care Coordinator (LVN)

About the role

Habitat Health is seeking a Home Care Coordinator (LVN) to ensure that personal and clinical home‑care needs are met for participants in the South Los Angeles PACE program. The role is part of an interdisciplinary team that assesses needs, develops care plans, and coordinates services to help older adults thrive at home.

Key responsibilities

• Conduct comprehensive in‑home assessments to evaluate care needs, functional status, safety risks, and suitability of home‑care services.
• Perform home safety evaluations and provide recommendations to promote independence and reduce risk.
• Assess participants’ ability to safely perform tasks and use adaptive equipment through observation and return demonstrations.
• Collaborate with participants, caregivers, and family members to identify care needs and service gaps.
• Participate actively in the Interdisciplinary Team (IDT) to develop, implement, and update participant care plans.
• Partner with clinical and non‑clinical staff—including rehabilitation, nutrition, and social services—to ensure a comprehensive understanding of participant needs.
• Coordinate home‑care services to align with individualized care plans and goals.
• Review weekly home‑care notes and documentation to identify service gaps, changes in condition, and service determination requests.

Required profile

• Licensed Vocational Nurse (LVN) certification.
• Experience conducting home assessments and safety evaluations.
• Ability to work collaboratively with interdisciplinary teams, participants, caregivers, and families.
• Strong observational skills to assess functional abilities and equipment use.
