Group Support Facilitator

About the role

Carda Health is seeking a skilled Group Support Facilitator to lead virtual group sessions for cardiac rehabilitation patients. You will be a core member of a transdisciplinary team delivering a remote, data‑driven Intensive Cardiac Rehab program.

Key responsibilities

• Lead emotionally safe group support sessions following Carda’s clinical protocols.
• Provide education during lifestyle classes and troubleshoot patients’ technical issues with wearables and tablets.
• Collaborate with founders and the technology team to improve the digital care delivery model.
• Support patients in taking ownership of their rehabilitation and maintain a basic understanding of nutrition, stress management, and exercise components.

Required profile

• Minimum 2 years of clinical or facilitation experience in behavioral health, cardiopulmonary, or chronic disease settings.
• Licensed clinical social worker, licensed professional counselor, or equivalent mental‑health credential.
• Master’s degree in social work, counseling, or a related mental‑health field.
• Self‑driven and able to work with little direction in a high‑growth startup environment.

Required skills

• Proficiency with mobile technology, wearables, and tablets for guiding patients through digital health programs.
• Strong interpersonal and active‑listening skills to create a compassionate virtual environment.
