Geriatric Nurse Practitioner - Lakewood/Westminster

About the role

An independent group practice of 13 physicians and one geriatric nurse practitioner is seeking a geriatric nurse practitioner to join its established outpatient geriatric clinic. The position splits the practice days between the Lakewood and Westminster offices, with no call obligations. It is a full-time position, four days a week.

Key responsibilities

• Perform a comprehensive patient health assessment, including health history and annual wellness visits
• Order, administer and analyze diagnostic tests
• Perform advanced technical procedures
• Set up systematic follow-up for the plan of care, reassess and modify it as needed, and initiate referrals and consult with physicians and other providers
• Maintain accurate, legible and confidential records
• Train patients and families on managing illness or injury and disease prevention
• Build a good working relationship with the healthcare team

Required profile

• Current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing and the Colorado Medical Board
• Valid prescriptive authority in the state of Colorado
• Current DEA certification
