Geriatric Nurse Practitioner - Lakewood/Westminster

About the role

An independent group practice of 13 physicians and one geriatric nurse practitioner is seeking a geriatric nurse practitioner to join its established outpatient geriatric clinic. The position splits the practice days between the Lakewood and Westminster offices, with no call obligations. The employer describes itself as a Top Place to Work for over a decade.

Key responsibilities

• Perform a comprehensive patient health assessment, including health history and annual wellness visits.
• Order, administer and analyze diagnostic tests.
• Perform advanced technical procedures.
• Set up systematic follow-up for the plan of care, reassess and modify it as needed, and initiate referrals and consult with physicians and other providers.
• Maintain accurate, legible and confidential records.
• Train patients and families on managing illness or injury and disease prevention.
• Build a good working relationship with the healthcare team.

Required profile

• Current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing and the Colorado Medical Board.
• Valid prescriptive authority in the state of Colorado.
• Current DEA certification.
